Does ultrasound therapy effectively treat injuries?

Is there evidence or clinical trials that prove ultrasound is an effective treatment for injuries? If physical therapists have been using ultrasound for 60 years and receive payment through medical insurance for it, I assume there are clinical trials that prove its effectiveness. I guess it’s good for pain and healing. I found information here ( electrotherapy.org/modalities – under ultrasound ) but it is very technical and sometimes confusing. There are significant clinical trials and studies here ( doesultrasoundtherapywork.blogspot.com ) but not enough, and surprisingly, all the studies seem to say it has a placebo effect, except for one. Some PTs and athletic trainers say its only good for some injuries, but which ones? – e.g. Knee injuries, shoulder injuries, musculoskeletal injuries, tendinitis, tendon tears, bone injuries. Can you please address the studies found on these sites and find studies that support ultrasound therapy? Even though you may be a PT, I would still like to see some evidence – no offense intended. Thanks for reading!
Also ankle sprains, etc.
I see that a PT did exactly what I asked not to do, instead of addressing the studies, he just stated what theoretically happens when ultrasound is applied to tissue. As you can see from my research, this is not sufficient. Address the clinical trials, if you would. Thanks.
Mistify: Better answer, but still I’m looking for specific studies, and you are right, it is a broad question. I’ll look at the pedro place you said to go. However, the second place I cited above ( doesultrasoundtherapywork.blogspot.com ) seems to list strong evidence against ultrasound therapy.
Mistify & Anyone Interested: It seems, you didn’t look close enough at the articles. 6/11 articles are legitimate clinical trials, while the other 5 are pretty decent systematic reviews. For example, the 3rd article deemed 10 out of 35 clinical trials fit to review, 2 supported ultrasound, while 8 were against it. (I bet those two were good one’s two)
I definitely don’t take anything at face value. I checked, all the articles, except for that random Nigerian one, are obviously from good sources (Oxford, Cochran, JAPT). After some of the clinical trials, you are right that they say more studies should be done. Yet, that does not discount their clinical trials. I don’t see any trials supporting ultrasound, which can’t be! Mistify, I did a search on Pedro, and came up with the same studies that are listed on that page.
I think it is the way I’m looking at it. Correct me if I’m wrong, but I’m starting to conclude that once Doctors charge cash for a treatment (take money from hard working
people) they should have strong evidence to support that treatment, or it is unregulated like the sham elixirs of the early 1900s. As a runner, I’m holding out hope that ultrasound works, but the evidence seems to the contrary.

Team Leader answers:

The debate on US under the parameters you have give could lead one to discuss this for days on end without any real answers. To say the very least, US may be marginally better than no treatment at all for a variety of conditions. However, the trouble with most studies on ultrasound are either A: poorly done, or B: focus on a very narrow and specific group of patients. To say the very least, you may want to narrow your question because it’s quite possible that more definitive answers will be there for more specific questions (ie…looking at it for very specific conditions). PEDro can help you sort through that…

I’d recommend you go to the Pedro database. It indexes about 480 studies on ultrasound…the first hundred or so are systematic reviews, so if you want clinical trials, you’ll need to scroll to about 100. It also states the quality of the trial and provides links to study abstracts when possible. You’ll also need to sort through clinical ultrasound (delivered in a physical therapy setting) and low intensity long duration ultrasound (as in bone stimulators that are worn for several hours at a time).

Some quick snippets, however…
1. Not effective for neck pain
2. It might work in hip arthritis (but only a small study was conducted)
3. Doesn’t do much for delayed onset muscle soreness
4. At the doses tested, not beneficial for ankle sprains
5. Does not add benefit to exercise therapy for shoulder problems (ie, exercise alone was sufficient)
…after that, the quality of the studies really drops off…

Sorry I can’t answer your question, but it’s a pretty broad question. Generally speaking, however, US lacks a lot of GOOD evidence…for or against it. Lack of evidence for it’s use is certainly better than strong evidence AGAINST it’s use.

….The website lists many systematic reviews, which are not clinical trials themselves, but rather review overall results of many studies. This can be a difficult task and depends a lot on the quality of the studies themselves. If you actually go in a read these reviews (not just the abstracts), the authors almost always state that their review is limited by poor quality of existing studies and parameters that make it difficult to extrapolate useful information. This is MUCH different than strong evidence AGAINST the use of US. Rather, there is CONFLICTING evidence as to whether US is effective or not. Namely, that there are a FEW moderate studies supporting it, and a FEW moderate studies supporting against it’s use.

…My personal feeling…I find it of little benefit.

Treatment for sprained ankle and tendinitis not working?

Over a year ago, my left ankle started hurting. I have no idea what has caused it. I went to the Dr for it 12 months ago. He said I wasnt limping, I was fine. He gave me meds for tendinitis and I never went back to him due to the way I was treated. Now my ankle is killing me. I can walk with tolerant pain,
When I move my left foot down and inward, the ankle hurts so bad and sends a sharp pain towards my toes. even something as minor as the blanket being pulled over my foot the wrong way makes me want to cry.

Ive done the rice treatment- no help, Ive tried an ace bandage and an ankle brace- no help because i can still move my ankle

Im seriously at my wits end over this. I hate to go back to the Dr only to be called a faker. Does anyone else have any suggestions please?
Seeing a different Dr would be fine, but would they need the records from the Dr I already went to?

I’m not positive, but i really don’t think they’d need the records. You can just tell them that you went to see someone else and they gave less than stellar treatment and you want a second opinion.

Ive had back pain due to Sports injuries. Can Marijuana be a treatment?

Hello! Im soon to begin school in Sacramento, CA. Im 19 years old and Ive played Baseball for about 15 years. But after playing for so long i have had many injuries. Shoulder injuries, knee, and back. I used to be a Catcher to those familiar with the sport and when your kneeling down behind the plate it gets pretty rough. But this year i just couldn’t take it because of my back and knee. Ive had to go to Chiropractors since i was 13 for adjustments but i always found myself back in his office because of baseball. I have recently tried marijuana through vaporizing and every time i would smoke, my back would loosen up and im able to relax. not only that, ive had less stress and better sleep. I don’t have a cannabis card yet but i feel that smoking marijuana really helps out. i was wondering what every ones thought is on my situation. This is a serious question and i please request serious answers.
Thank you.
Because of school, i sadly wont be playin baseball anymore. My knee and shoulder injuries are fairly recent injuries but ive had back problems since i was around 13. Ive been told by my chiropractor that i do special back stretchs but it doesnt fix it. during the day my back will just ache.

Lately, I have had serious elbow pain, particularly when my arm is bent. Is there a way to fix this problem aside from stopping lifting altogether?

Is there something about my workout routine in particular that caused my problem? Too much pushing or pulling movement? Should I mix up the routine? Or is it just overuse in general? Seems to flare up most during chest exercises.

My routine is as follows, and has been the same for the entire 2-3 years that I’ve been lifting:

Chest:
40 pushups to warm up
Flat barbell bench press, 3 sets (one light warmup, two heavy)
15 degree incline dumbell press, 2 sets
30 degree incilne dumbell press, 2 sets
45 degree incline dumbell press, 2 sets

Lats:
Wide grip pull ups (palms facing out) to warm up
Wide grip pull ups with added weight, 2 sets
T-bar rows, 3 sets
Bent dumbbell rows, 2 sets

Shoulders:
Side lateral raises, 3 sets
Smith machine shoulder press, 3 sets
Upright rows, 3 sets
Shrugs, 2 sets

I don’t do any isolated arm exercises.

Do soem simple streching exercises and take some advil or alive for the pain. Depending on our weight and what not you can take up to 3 advil. I’m 5’2 and I weigh 118lbs my doctor says IM OK to take 3 once then 2 every 6 hours. It helps…as for everything you do..you’re nuts. Im sure you look great but maybe take it a little easy on your elbow, knees and sholder..you can couse tares if you over strain it.

Roos, I am going to ask you to try this. Instead of doing all of the movements that you have been doing switch to the uninjured side. I would like you to do this exercise. Sit in a chair and hold a weight that you are comfortable with in your uninjured sides hand. Raise that weight over your head 10 times and then rest 1 minute. Now take either that weight again or a weight that is slightly higher and do the same 10 lifts again. Rest 1 minute and then using the same weight or a heavier weight do another 10 lifts. Now take the lightest weight and do the same 10 reps on the injured side and upon finishing rest 1 minute. If you can use a heavier weight use it but if you are not able to stay with the original weight and do another 10 reps. Rest for the 1 minute and then do a last set of lifts. You should notice a difference in your ability to lift doing it this way. You can also do bicep curls and chest flies doing this method always starting with the uninjured side first.

Well the past few days my shoulder has been making some popping and snapping sensations. Its a little sore and feels a bit tight but not too painful. I’ve been considering wearing my sling for a few days and doing ice therapy and some of the exercises I did before. is this a good or bad idea? I’m just wondering if the popping with movement is a warning sign. I don’t want to go to the doc if I can help it. I will have to establish care with a new orthopedic doc. the symptoms are mostly an annoyance

Team Leader answers:

Onset of popping or snapping can be an indicator of soft tissue damage. You said you nearly dislocated your shoulder? What caused the near dislocation? Did the doc do an MRI? There is a chance that you may have damaged the cartilage in the shoulder socket and that is what is popping. If that is the case it will only get worse with time.

Pitchers that had Tendinitis, Come in…?

Ive had shoulder tendinitis a year ago, and met with a doctor. she sent me to a physical therapist, and she sounded as if it wasn’t anything that big. To me it’s something that has really changed my life. I can no longer pitch the way I did. To be honest, I haven’t bee doing the exercises that she assigned, I do them once In a while, but don’t you think the rest Ive given to my arm (months) my shoulder would’ve be fine by now? My shoulder is still not fine, because as I throw I can here a small cracking sound and the hand sort of separates from my body (No its not a labrum problem, just had MRI)

Anyways, the pitchers that have gone through tendinitis, are you able to throw now? how long did the healing take you.

Team Leader answers:

I was a starting pitcher for Division I college for four years. Whenever I had a bout with tendinitis I found a few things helped. Going on long slow runs to flush all the lactic acid out of your body is a good start. It isn’t a hard pace, if you run with someone else, you should be able to carry on a conversation as you run. The second thing is doing the exercises prescribed to you and do them religously. If they were the cord and 5lb weight drills you need to stay on top of those. I did these even when I wasn’t hurt and it helped me make it through long seasons pitching over 100 innings. Last, if it still doesn’t go away, IB profen and Glucosamine Chondroitin will help take away any inflamation.

Right arm pain..how to get rid of it?

I got injured on the job at the end of May. To be specific, my right wrist. My doc said it was tendonitis. To make a long story short…I was out for 3 months during that time I kept my wrist in a splint type brace. About half way through the 3 months my elbow, forearm and shoulder started hurting also. I took IBprofen everyday and I iced it every night and kept wearing the brace. The beginning of Aug I had a 2nd opinion apt. And that doc said the brace was causing the extra pain and to not wear it anymore.
So my question is It’s now been a month since I stopped wearing the brace, it doesn’t hurt as much or as frequently but it DOES still hurt. It’s been almost 4 months.
What can I do to make it better???
I’ve been out of work for 3 months and for the past 3 weeks I haven’t been getting paid. I need a new job, but don’t want to re-injure myself.
Any stretches I can do? (I know the pull wrist stretches). Weight lifting excersises?
Do you think a bad mattress adds to it?
Maybe the way I sleep?
I’ve been to 3 kaiser doctors, 2 workers comp doctors, and a physical therapy doctor.
I’ve had x-rays it’s not bones, i don’t need surgery. I believe it is tendonitis like ALL of them told me, it just isn’t going away after following everything they’ve told me to do. And I’m tired of taking ibprofen everyday

Team Leader answers:

Where’s worker’s comp?

Anyhow, good for you, taking off the splint. It will strenghten and bring back some flexibility.

See a physiotherapist and charge it to the former job/employer/health plan.

You need appropriate mild exercises and another opinion about the bed (it can affect you) which is difficult, but probable if you explain the whole thing.

Take this seriously, tendinitis can last for years, and I don’t mean carpal tunnel syndrome.
