<h2>Shoulder and neck medical question?</h2>
<p>I&#8217;ve been having <strong>shoulder</strong> pain for 3 years now and the doctor thought it was <strong>tendonitis</strong>, so i went to <strong>physical</strong> <strong>therapy</strong> and it went away. Now it&#8217;s come back and its worse than ever. It spread to my neck and i get horrible headaches. I got an x-ray and it shower nothing, and the MRI showed nothing. the doctor thinks my <strong>shoulder</strong> is just weak but i dont think that&#8217;s the problem. do you have any clue what it could be?</p>

<h2>Question about dislocated shoulder and nerve damage?</h2>
<p>I suffered a left <strong>shoulder</strong> dislocation on 06-03-09, after falling very hard. I had an MRI done, and was shown to have torn my <strong>rotator</strong> <strong>cuff</strong> and labrem, as well as a slight fracture on the socket as well. I&#8217;ve seen an Ortho doctor, and I have 2 questions. I am not in  a lot of pain, and I actually have really good range of movement. But, I am numb from the top of that <strong>shoulder</strong>, to almost my elbow. It feel like my arm has fallen asleep, or is &#8220;dead&#8221;. My ortho says, 99% of the time, the feeling will come back. He told me to wait 3 weeks till my next appt, and all should be back to normal. Well, 3 weeks is approaching in a few days, and I am still numb. Is this a normal nerve reaction from an <strong>injury</strong> like this, or can it mean permanent nerve damage? What is the <strong>treatment</strong> if the numbness does not subside? Also, he says surgery is definitely required, but he told me a simple scope will not work, he needs to cut me. Any idea why a scope would not work for a <strong>shoulder</strong> repair? Thanks!</p>

<h2>When frozen shoulder (or &#8220;adhesive capsulitis&#8221;) goes away &#8211; does it just disappear?</h2>
<p>I have a <strong>frozen</strong> <strong>shoulder</strong> &#8211; and have had it since last September.  I went through physical therapy &#8211; and do <strong>exercises</strong> daily.  It&#8217;s much better than it was before the <strong>treatment</strong> and <strong>exercises</strong> &#8211; but it still bothers me.  I&#8217;ve read that it just &#8220;goes away&#8221; &#8211; after a year &#8211; or maybe two &#8211; or even three.</p>
<p>Has anyone had it &#8211; and had it go away?  How does that work?  Do you just wake up one morning and it&#8217;s no longer <strong>frozen</strong>?  Or is it a gradual thing?</p>
<p>Would really appreciate input.  I&#8217;m really sick of doing the <strong>exercises</strong> &#8211; and putting up with it &#8211; and i want to know what I can be looking forward to in regard to recovery.</p>

<h2>Does ultrasound therapy effectively treat injuries?</h2>
<p>Is there evidence or clinical trials that prove ultrasound is an effective <strong>treatment</strong> for injuries? If physical therapists have been using ultrasound for 60 years and receive payment through medical insurance for it, I assume there are clinical trials that prove its effectiveness. I guess it&#8217;s good for pain and healing. I found information here ( electrotherapy.org/modalities &#8211; under ultrasound ) but it is very technical and sometimes confusing. There are significant clinical trials and studies here ( doesultrasoundtherapywork.blogspot.com ) but not enough, and surprisingly, all the studies seem to say it has a placebo effect, except for one. Some PTs and athletic trainers say its only good for some injuries, but which ones? &#8211; e.g. Knee injuries, <strong>shoulder</strong> injuries, musculoskeletal injuries, tendinitis, tendon tears, bone injuries. Can you please address the studies found on these sites and find studies that support ultrasound therapy? Even though you may be a PT, I would still like to see some evidence &#8211; no offense intended. Thanks for reading!<br />

I see that a PT did exactly what I asked not to do, instead of addressing the studies, he just stated what theoretically happens when ultrasound is applied to tissue. As you can see from my research, this is not sufficient. Address the clinical trials, if you would. Thanks.<br />
Mistify: Better answer, but still I&#8217;m looking for specific studies, and you are right, it is a broad question. I&#8217;ll look at the pedro place you said to go. However, the second place I cited above ( doesultrasoundtherapywork.blogspot.com ) seems to list strong evidence against ultrasound therapy.<br />
Mistify &amp; Anyone Interested: It seems, you didn&#8217;t look close enough at the articles. 6/11 articles are legitimate clinical trials, while the other 5 are pretty decent systematic reviews. For example, the 3rd article deemed 10 out of 35 clinical trials fit to review, 2 supported ultrasound, while 8 were against it. (I bet those two were good one&#8217;s two)<br />
I definitely don&#8217;t take anything at face value. I checked, all the articles, except for that random Nigerian one, are obviously from good sources (Oxford, Cochran, JAPT). After some of the clinical trials, you are right that they say more studies should be done. Yet, that does not discount their clinical trials. I don&#8217;t see any trials supporting ultrasound, which can&#8217;t be! Mistify, I did a search on Pedro, and came up with the same studies that are listed on that page.<br />
I think it is the way I&#8217;m looking at it. Correct me if I&#8217;m wrong, but I&#8217;m starting to conclude that once Doctors charge cash for a <strong>treatment</strong> (take money from hard working<br />
people) they should have strong evidence to support that <strong>treatment</strong>, or it is unregulated like the sham elixirs of the early 1900s. As a runner, I&#8217;m holding out hope that ultrasound works, but the evidence seems to the contrary.</p>

<p class="dtm-content">The debate on US under the parameters you have give could lead one to discuss this for days on end without any real answers.  To say the very least, US may be marginally better than no treatment at all for a variety of conditions. However, the trouble with most studies on ultrasound are either A: poorly done, or B: focus on a very narrow and specific group of patients.  To say the very least, you may want to narrow your question because it&#8217;s quite possible that more definitive answers will be there for more specific questions (ie&#8230;looking at it for very specific conditions).  PEDro can help you sort through that&#8230;</p>

<p>I&#8217;d recommend you go to the Pedro database.  It indexes about 480 studies on ultrasound&#8230;the first hundred or so are systematic reviews, so if you want clinical trials, you&#8217;ll need to scroll to about 100. It also states the quality of the trial and provides links to study abstracts when possible.  You&#8217;ll also need to sort through clinical ultrasound (delivered in a physical therapy setting) and low intensity long duration ultrasound (as in bone stimulators that are worn for several hours at a time).  </p>
<p>Some quick snippets, however&#8230;<br />
1. Not effective for neck pain<br />
2. It might work in hip arthritis (but only a small study was conducted)<br />
3. Doesn&#8217;t do much for delayed onset muscle soreness<br />
4. At the doses tested, not beneficial for ankle sprains<br />
5. Does not add benefit to exercise therapy for shoulder problems (ie, exercise alone was sufficient)<br />
&#8230;after that, the quality of the studies really drops off&#8230;</p>
<p>Sorry I can&#8217;t answer your question, but it&#8217;s a pretty broad question.  Generally speaking, however, US lacks a lot of GOOD evidence&#8230;for or against it.  Lack of evidence for it&#8217;s use is certainly better than strong evidence AGAINST it&#8217;s use.</p>
<p>&#8230;.The website lists many systematic reviews, which are not clinical trials themselves, but rather review overall results of many studies.  This can be a difficult task and depends a lot on the quality of the studies themselves.  If you actually go in a read these reviews (not just the abstracts), the authors almost always state that their review is limited by poor quality of existing studies and parameters that make it difficult to extrapolate useful information.  This is MUCH different than strong evidence AGAINST the use of US.  Rather, there is CONFLICTING evidence as to whether US is effective or not.  Namely, that there are a FEW moderate studies supporting it, and a FEW moderate studies supporting against it&#8217;s use.  </p>
<p>&#8230;My personal feeling&#8230;I find it of little benefit.</p>

<h2>Competing: horse with old shoulder injury?</h2>
<p>I would like your views on this issue please.</p>
<p>I have been schooling/getting fit a horse for a friend since september 08. He is about 11-15yrs old, about 16.1hh, and his beeding is Russian Don. My friend has had him just over 4 years now. I&#8217;ve been doing the normal schooling and hacking exercises as well as lunging. He&#8217;s going well and when the ground is better i&#8217;ll start some jumping.</p>
<p>What she&#8217;s been able to piece together is this: As a youngster he was trained as a showjumper (pretty top level as far as we can tell) <strong>in</strong> russia. He was then transproted to England and thats where we think he had his accident. He managed to badly damage his <strong>shoulder</strong> (muscles/ligaments rather than bone it seems) as he was being unloaded (?) we think. This meant his showjumping career was over. My friend then saw him <strong>in</strong> a field where her friend worked. He had been left <strong>in</strong> this field for a year we think, pretty much ignored. To cut a long story short my friend ended up buying him and she&#8217;s had him since. He&#8217;s been <strong>in</strong> light work since, doing a bit of schooling, hacking and jumping. </p>
<p>This is the problem: He is not sound <strong>in</strong> the walk (his left fore stride is slightly shorter), starts off short <strong>in</strong> the trot but with work and warming up you&#8217;d never notice <strong>in</strong> the trot and canter. He does not seem to experience any pain and hasnt for a long as my friend has owned him (been vetted etc and this is constantly checked.) He has absolutely beautiful paces when he moves and obviously has been very well schooled.<br />
I have spoken to various people, and the issue seems to be this- how can i keep his life interesting when its obvious he wont be allowed to do any competions. every one would say he was lame. What can we do? I would love to be able to take him to do some dressage and sj even xc competitons as he loves doing work, but what is the option when a horse has an old injury which means he&#8217;ll never be &#8216;sound&#8217;!<br />
just to clarify- he experiences no pain whatsoever. the vet has told us this. his movement is short due to the damage done to his muscles, just like <strong>in</strong> a human- scar tissue.</p>

<p>If it&#8217;s just local shows you could give him half a bute before he goes.  But you cant comepte him on painkillers.</p>

<h2>Had Shoulder injury from Pushups, now better but still feel slight pain. is it healing?</h2>
<p>I did flying pushups.. you konw, where you jumt up with your hands and do like a clap, then land. Then the next day i was in bad pain. I had bad pain for about 3 days. thefirst days the pain was constant,  third day the pain was more intermitent. All the while.. i never had problem with the range of motion.. i could lift my arm and never felt like i couldn&#8217;t move it.. </p>
<p>ANyways, i still have full range of motion.. but now, 5 days later i have minimal pain.. sometimes i dont even have any pain.. but there&#8217;s a slight weird feeling in my <strong>shoulder</strong> somewhere inside. </p>
<p>Dont have any problems moving it. Anyways, when i dont have pain i kind of feel close to normal. but when i do have pain in a scale <strong>from</strong> 1 to 10 , the pain is now at 2. while at the beginning it was at 9</p>
<p>Again, it&#8217;s been 5 days. I&#8217;ve had plenty of sprains in my life.. Some took like a year to heal but they did. some never healed but they stopped hurting .. etc. So is there a chance this will at least heal good enough?<br />
By the way,, this is not like pain <strong>from</strong> sore muscles. Not at all. It&#8217;s some weird pain inside the soulder. So that&#8217;s why i&#8217;m asking</p>

<p class="dtm-content">Give it time enough to heal.  Muscles are attached to bones by tendons.  Bones are attached to other bones with ligaments.<br />
Either way, these bands are similar, but stronger than, rubber bands.  They only stretch so far before they &#8220;tear&#8221;.  Ouch!!!</p>
<p>Once they are torn, they have to grow back together and  must be completely rested for a few days while swallowing ibuprophen for pain and applying heat as much as possible.</p>
<p>After a few days one can begin slowly moving the injured area in a normal manner.  The area must not be &#8220;worked&#8221; or used beyond what is just normal wear/tear.  In other words, no exercise.   That will &#8220;work&#8221; the area.  You will ease into a slight work schedule where you will move the injured area above and beyond normal activity as you slowly strengthen the area again.</p>
<p>Now, that is what one should do to allow an injury to heal fully.  Depending on the injury and it&#8217;s location, this process can indeed take up to a year.  To feel pain is the body&#8217;s way of telling you something is wrong.  If you don&#8217;t hurt any more, then you should be healed.</p>

<p>Is there evidence or clinical trials that prove ultrasound is an effective <strong>treatment</strong> for injuries? If physical therapists have been using ultrasound for 60 years and receive payment through medical insurance for it, I assume there are clinical trials that prove its effectiveness. I guess it&#8217;s good for pain and healing. I found information here ( electrotherapy.org/modalities &#8211; under ultrasound ) but it is very technical and sometimes confusing. There are significant clinical trials and studies here ( doesultrasoundtherapywork.blogspot.com ) but not enough, and surprisingly, all the studies seem to say it has a placebo effect, except for one. Some PTs and athletic trainers say its only good for some injuries, but which ones? &#8211; e.g. Knee injuries, <strong>shoulder</strong> injuries, musculoskeletal injuries, tendinitis, tendon tears, bone injuries. Can you please address the studies found on these sites and find studies that support ultrasound therapy? Even though you may be a PT, I would still like to see some evidence &#8211; no offense intended. Thanks for reading!<br />

I see that a PT did exactly what I asked not to do, instead of addressing the studies, he just stated what theoretically happens when ultrasound is applied to tissue. As you can see from my research, this is not sufficient. Address the clinical trials, if you would. Thanks.<br />
Mistify: Better answer, but still I&#8217;m looking for specific studies, and you are right, it is a broad question. I&#8217;ll look at the pedro place you said to go. However, the second place I cited above ( doesultrasoundtherapywork.blogspot.com ) seems to list strong evidence against ultrasound therapy.<br />
Mistify &amp; Anyone Interested: It seems, you didn&#8217;t look close enough at the articles. 6/11 articles are legitimate clinical trials, while the other 5 are pretty decent systematic reviews. For example, the 3rd article deemed 10 out of 35 clinical trials fit to review, 2 supported ultrasound, while 8 were against it. (I bet those two were good one&#8217;s two)<br />
I definitely don&#8217;t take anything at face value. I checked, all the articles, except for that random Nigerian one, are obviously from good sources (Oxford, Cochran, JAPT). After some of the clinical trials, you are right that they say more studies should be done. Yet, that does not discount their clinical trials. I don&#8217;t see any trials supporting ultrasound, which can&#8217;t be! Mistify, I did a search on Pedro, and came up with the same studies that are listed on that page.<br />
I think it is the way I&#8217;m looking at it. Correct me if I&#8217;m wrong, but I&#8217;m starting to conclude that once Doctors charge cash for a <strong>treatment</strong> (take money from hard working<br />
people) they should have strong evidence to support that <strong>treatment</strong>, or it is unregulated like the sham elixirs of the early 1900s. As a runner, I&#8217;m holding out hope that ultrasound works, but the evidence seems to the contrary.</p>

<p class="dtm-content">The debate on US under the parameters you have give could lead one to discuss this for days on end without any real answers.  To say the very least, US may be marginally better than no treatment at all for a variety of conditions. However, the trouble with most studies on ultrasound are either A: poorly done, or B: focus on a very narrow and specific group of patients.  To say the very least, you may want to narrow your question because it&#8217;s quite possible that more definitive answers will be there for more specific questions (ie&#8230;looking at it for very specific conditions).  PEDro can help you sort through that&#8230;</p>

<p>I&#8217;d recommend you go to the Pedro database.  It indexes about 480 studies on ultrasound&#8230;the first hundred or so are systematic reviews, so if you want clinical trials, you&#8217;ll need to scroll to about 100. It also states the quality of the trial and provides links to study abstracts when possible.  You&#8217;ll also need to sort through clinical ultrasound (delivered in a physical therapy setting) and low intensity long duration ultrasound (as in bone stimulators that are worn for several hours at a time).  </p>
<p>Some quick snippets, however&#8230;<br />
1. Not effective for neck pain<br />
2. It might work in hip arthritis (but only a small study was conducted)<br />
3. Doesn&#8217;t do much for delayed onset muscle soreness<br />
4. At the doses tested, not beneficial for ankle sprains<br />
5. Does not add benefit to exercise therapy for shoulder problems (ie, exercise alone was sufficient)<br />
&#8230;after that, the quality of the studies really drops off&#8230;</p>
<p>Sorry I can&#8217;t answer your question, but it&#8217;s a pretty broad question.  Generally speaking, however, US lacks a lot of GOOD evidence&#8230;for or against it.  Lack of evidence for it&#8217;s use is certainly better than strong evidence AGAINST it&#8217;s use.</p>
<p>&#8230;.The website lists many systematic reviews, which are not clinical trials themselves, but rather review overall results of many studies.  This can be a difficult task and depends a lot on the quality of the studies themselves.  If you actually go in a read these reviews (not just the abstracts), the authors almost always state that their review is limited by poor quality of existing studies and parameters that make it difficult to extrapolate useful information.  This is MUCH different than strong evidence AGAINST the use of US.  Rather, there is CONFLICTING evidence as to whether US is effective or not.  Namely, that there are a FEW moderate studies supporting it, and a FEW moderate studies supporting against it&#8217;s use.  </p>
<p>&#8230;My personal feeling&#8230;I find it of little benefit.</p>

<p>how ever they said i had flat feel and new running shoes and this <strong>treatment</strong> should help<br />
it hasnt worked at all<br />
 with in 10 minutes of standing at my gas stataion job i start to feel the pain and see the swelling it gets so bad that by the end of the night i can hardly walk<br />
the doctors told me even with flat feet juststanding there should be causing such pain and swelling<br />
running would cause this but if i had got new shoes itll go away; never has.</p>
<p>now i feel like its getting worse i can hardly touch my legs and i think its starting in my thighs too</p>
<p>WHAT THE HELL IS GOING ON WHY IS THIS HAPPENING<br />
should i see a new doc or a new PT ?<br />
are there any specialists?</p>

Now there are support hoses that you can pick up at any rite-aid or walgreens. I would get another doctors opinion though if u can</p>

<h2>Treatment for sprained ankle and tendinitis not working?</h2>
<p>Over a year ago, my left <strong>ankle</strong> started hurting. I have no idea what has caused it. I went to the Dr for it 12 months ago. He said I wasnt limping, I was fine. He gave me meds for tendinitis and I never went back to him due to the way I was treated. Now my <strong>ankle</strong> is killing me. I can walk with tolerant pain,<br />
When I move my left foot down and inward, the <strong>ankle</strong> hurts so bad and sends a sharp pain towards my toes. even something as minor as the blanket being pulled over my foot the wrong way makes me want to cry.</p>
<p>Ive done the rice <strong>treatment</strong>- no help, Ive tried an ace bandage and an <strong>ankle</strong> brace- no help because i can still move my <strong>ankle</strong></p>
<p>Im seriously at my wits end over this. I hate to go back to the Dr only to be called a faker. Does anyone else have any suggestions please?<br />
Seeing a different Dr would be fine, but would they need the records from the Dr I already went to?</p>

<p>I&#8217;m not positive, but i really don&#8217;t think they&#8217;d need the records.  You can just tell them that you went to see someone else and they gave less than stellar treatment and you want a second opinion.</p>

I used to be a Catcher to those familiar with the sport and when your kneeling down behind the plate it gets pretty rough. But this year i just couldn&#8217;t take it because of my back and knee. Ive had to go to Chiropractors since i was 13 for adjustments but i always found myself back in his office because of baseball. I have recently tried marijuana through vaporizing and every time i would smoke,  my back would loosen up and im able to relax. not only that, ive had less stress and better sleep. I don&#8217;t have a cannabis card yet but i feel that smoking marijuana really helps out. i was wondering what every ones thought is on my situation. This is a serious question and i please request serious answers.<br />

Because of school, i sadly wont be playin baseball anymore. My knee and <strong>shoulder</strong> injuries are fairly recent injuries but ive had back problems since i was around 13. Ive been told by my chiropractor that i do special back stretchs but it doesnt fix it. during the day my back will just ache.</p>
